The documentation and forms on this page are intended for use by parties that have a current Pole License Agreement or Joint Use Agreement in place with Avista.
General Program Information
The Avista Joint Use Policies and Procedures 2024 book (PDF) includes all relevant forms, guidelines, flow-charts, and regulations needed to apply for and construct pole attachments on Avista structures. For Avista to process and approve pole attachment applications in the most timely and efficient way possible, we encourage all licensees to thoroughly review and refer to this document.
Pole Attachment Requests (“Route Application”)
Below is a checklist of the required forms and documents that must be submitted to be considered a complete application. Please note that these individual forms are also included in the Policies and Procedures book.
To apply, please submit the appropriate exhibit form (complete and signed) and your design plan (in PDF format) to JointUse@avistacorp.com.
- Application Form
- “Exhibit A” for new pole attachments
- “Exhibit A” for space in Avista underground duct/conduit
- “Exhibit B” for overlash notifications
- Engineered Design Plan (supplied by applicant)
Avista will confirm receipt of your application (if complete) or notify you of any errors or omissions. If complete, it will be assigned a route application number and a Joint Use Customer Project Coordinator (CPC) that will process your application.
Please direct all application questions to the CPC assigned to it, and always refer to the route application number it was assigned.
Application fees
- Wireline attachments: $600 Base Fee + $42 Per Pole
Fees will be invoiced to the licensee after the application is accepted. Please inquire about fees for duct space and wireless antenna attachments.
